Session 2: Leveraging OASIS to Capture Social Determinants in Home Health 

 Learning Objectives: 

  • Define social determinants of health and explain their significance in patient outcomes and health disparities. 
  • Discuss how SDOH can affect patient care, recovery, and overall well-being in the home health setting. 
  • Identify specific OASIS items that capture data related to social factors (e.g., living situation, access to food, transportation, social support). 
  • Discuss how OASIS data related to SDOH can influence care planning and decision-making. 
  • Share strategies for integrating SDOH data into care plans to address patient needs holistically and improve health outcomes. 
  • Discuss how the inclusion of social determinants in home health assessments may evolve and impact policy, care delivery, and reimbursement in the future.

MEET OUR SPEAKER:

Heather Curren – SimiTree

Heather has been in the home care industry for the past 30 years. She received her Bachelors Degree in Nursing from the University of Alabama in Birmingham in 1992. She has extensive field and case management experience in both home health and hospice. Heather has held various positions in home health and hospice agencies but performance improvement, staff development and appeals are her areas of expertise. She has a vast knowledge of state and federal regulatory guidelines in both home health and hospice. Although, education remains Heather’s passion in this rapidly changing home health and hospice industry. Heather holds certifications in both OASIS competency and ICD 10 coding in home health and hospice. She has authored a book on medical necessity for home health. Heather currently works as a Director of Education for SimiTree HC in the coding and OASIS division. She enjoys spending time with family, is an avid college football fan and enjoys time spent with her new grandson.
