Meet our speakers from the Alzheimer’s Association:
Sara Murphy, Vice President for Programs and Services for the Greater PA Chapter of the Alzheimer’s Association has been with the Association for close to 15 years and served as the Program Director for the Mississippi Chapter managing the programs and service delivery for all 82 counties of Mississippi prior to moving to PA in 2016 where she is currently managing the programs and services for the 59 out of the 67 counties of PA. Sara has a background in nutrition/wellness, intergenerational programming and gerontology. Sara has experience in caring for those individuals living with Alzheimer’s and other dementia as she has and worked in the long term care industry for 5 years with all levels of care from independent living residents to memory care prior to her work with the Association.
Melanie Smith, MSW, MPA, MA, Director of Care and Support at the Alzheimer’s Association Delaware Valley Chapter. In this role, Melanie leads the Alzheimer’s Association’s program delivery and growth strategy to reach and serve all communities in Southern Pennsylvania (8 counties – Berks, Bucks, Chester, Delaware, Lehigh, Montgomery, Northampton, and Philadelphia Counties), Southern New Jersey and the state of Delaware, and she leads a community-based team to further the Alzheimer’s Association’s impact in mission programs and services.
She previously held roles in education, outreach and advocacy for national non-profit organizations, and also NYC Health + Hospitals (New York City Health and Hospitals Corporation) in public education, outreach and communication.
Melanie has a Master’s in Social Work from Boston University, a Master’s in Health Policy and Management from New York University and a Master’s in Public Communication from American University in Washington, DC.
Melanie is honored and grateful to serve the Alzheimer’s Association and advance its mission – to lead the way to end Alzheimer’s and all other dementia – by accelerating global research, driving risk reduction and early detection, and maximizing quality care and support.

Jennifer began her home health career in a large privately owned agency after transitioning from Medical-Surgical Urology hospital nursing; however, she has worked for all types of agencies in multiple states. Her love for sharing knowledge and experience led her to clinical consulting, where she enjoys translating technical guidance into information that the busy home health professional can deploy to improve the efficiency and quality of care being delivered to the patients they serve.
Jennifer Osburn has led numerous home health educational seminars for state and national associations as well as specifically designed education training plans for agencies for the past 12 years. In addition to consulting across the US, she has trained staff for The Joint Commission and ACHC, has authored numerous industry blogs and articles, and has assisted home health software companies with solutions that ensure compliance with Medicare guidelines. She is a member of the National Association for Healthcare Quality and holds a Certified Professional in Healthcare Quality (CPHQ) certification. As a part of the HPS clinical consulting team, Tammy provides support and solutions to home health and hospice agencies across the country to achieve regulatory compliance. Tammy’s commitment to service and compassionate nursing care started 30 years ago as a home health nurse. Her experience includes successfully starting a Hospice QAPI program and serving as a Hospice Quality Manager. Tammy has experience as a Home Health Clinical Manager as well as case management of home health patients. Tammy has provided education and leadership for home health, hospice, and acute care settings throughout her nursing career. Her knowledge of Federal and State Regulations along with her clinical experience motivates her to continually seek solutions to the barriers of healthcare in all settings. Melinda A. Gaboury, COS-C, is Co-Founder and Chief Executive Officer of Healthcare Provider Solutions. With more than 30 years in home care, has over 22 years of executive speaking and educating experience, including extensive day-to-day interaction with home care and hospice professionals. She routinely conducts home care and hospice reimbursement workshops and speaks at state association meetings throughout the country. Melinda has extensive experience in Medicare Patient-Driven Groupings Model (PDGM) training, billing, collections, case-mix calculations, chart reviews, and due diligence. Medicare appeals (UPIC, RA, ADR, and TPE) with all the Medicare Administrative Contractors (MACs) have become the forefront of Melinda’s current impact on the industry. She is currently serving as the Chair of the National Association for Home Care & Hospice (NAHC) Home Care and Hospice Financial Managers Association (HHFMA) Advisory Board and Work Group, Treasurer of the Home Care Association of Florida Board of Directors and serves on the Tennessee Association for Home Care Board. Melinda is also the author of the Home Health OASIS Guide to OASIS-E and Home Health Billing Answers 2024.
